What should I bring to my first G2 Orthopedics appointment?

Please review the list below for items to bring to your first G2 Orthopedics appointment. While some patients like to complete new patient paperwork prior to their visit, we advise all patients to still bring the items listed below. 

First G2 Orthopedics Appointment Patient Must Haves:

• Insurance card
• Photo ID
• List of medications
• If you complete and print the New patient paperwork  it will save time at the appointment
• Prior X-Ray or MRI if for the same body part – Don’t worry if you don’t have these, we can still see you and obtain new x-rays if required.
