This notice is being made in accordance with Virginia Code § 54.1-2405 with regard to the transfer of records in conjunction with the closure of a professional practice.

At the written request of a patient or an authorized representative (with the date and signature of such patient or authorized representative), a patient’s records or copies thereof will be sent, within a reasonable time, to any other like-regulated provider of the patient’s choice or provided to the patient pursuant to Virginia Code § 32.1-127.1:03. Patients will be charged for the actual costs of copying and mailing or delivering the records to the patient or to another like-regulated provider of the patient’s choice.
